  1. If you decide to reactivate your enrollment in the future (and we hope you do) enrollment rates in effect at the time of reactivation will be applicable as enrollment rates are subject to change.
  2. If you are traveling or injured and would like to put your enrollment on hold instead of cancelling, you can submit an Enrollment Hold Request HERE.
  3. If you’re sure you’d like to cancel, just complete and submit the form below. This will serve as your 30-day written cancellation notice as required by your enrollment agreement. Note that if you have a scheduled renewal payment within this 30-day period, the payment will be processed as scheduled. All payments are non-refundable.
  4. Cancellation of enrollment due to moving away will not require an early termination fee, but 30 day written cancellation policy is still in force.
  5. Note that if you have a scheduled renewal payment within this 30-day period, the payment will be processed as scheduled at a pro-rate based on the date the form was submitted, or the email was received. All payments are non-refundable.
